Research Summary

This retrospective study examined a rapid, split-dose methadone initiation protocol for hospitalized pregnant patients with opioid use disorder. Fourteen patients received the protocol. Methadone doses increased over the first five days, with no serious adverse events reported; one patient’s titration was slowed because of asymptomatic bradycardia.

After discharge, 12 of the 14 patients attended an opioid treatment program intake, and eight remained on methadone at 30 days. The authors concluded that this approach may be feasible for carefully selected patients, while emphasizing the need for continued support after hospitalization.
